SERVICES
supported discharge
We help bridge the gap between inpatient stays and home.
HOW WE
Bridge the gap
We provide rapid patient pick-up and intensive rehabilitation programmes across all professions, ensuring a smooth transition from hospital to home with the right equipment and support.
KEY SERVICES
- Rapid Patient Pick-up: Begin rehabilitation as soon as the same day of discharge.
- Collaboration with Inpatient Teams: We work with your inpatient team to plan your discharge, attend discharge planning meetings, and, in some cases, arrange joint therapy sessions for a seamless handover.
- Intensive Rehab Programmes: Tailored therapy sessions, including physiotherapy, occupational therapy, speech and language therapy, psychology, and dietetics.
- Visits Around Care Commitments: Flexible scheduling to fit around care calls and other responsibilities.
- Specialist Equipment Hire: Immediate access to essential equipment, preventing discharge delays.
- Care Agency Partnerships: We coordinate with care agencies, offering 7-day support and continuity.
- Hospital Discharge Support: Occupational Therapists prepare your home for a safe and smooth return. Once you’re ready, therapy can continue in our clinics, rehab spaces, or hydrotherapy pools, with schedules tailored to your individual needs.
- Services for preventing inpatient admission : For patients at home in the community looking to avoid hospital admission, we offer therapy services designed to help prevent this. Our team provides intensive rehabilitation and support to manage your care needs at home, reducing the risk of hospitalisation and promoting recovery in a familiar environment.
